A form needs to be completed for each player using United Way Funds.  This form needs to be submitted to us then a registration code will be sent to you for you to register your player(s).  Please complete the form and mail along with income documents to NUUSC PO BOX 273, New Ulm, MN 56073.  We will complete the organization portion and mail to UW.
