2012 Registration Information
NUAYSA soccer is divided into three levels:
- Classic (Competitive Division) – U11 through U17
- Recreation-Plus (“Rec-Plus”) – U9, U10 and U19
- Recreational (“Rec”) – U4 to U8

Registration Deadlines
U11 - U17 Classic Registration Deadline is February 22, 2012.
U9, U10 and U19 RecPluc Registration Deadline is March 15, 2012.
U4 - U8 Rec Registration Deadline is April 13, 2012.
Age Division Verification
BEFORE you register your child, you must verify the level at which they can play. This can be done by entering the birth date of each player you are registering.

Registering Multiple Players
Maximum fees for one family are limited to $500 provided you register all players in your family at the same time.
To register more than one player in your family:
- ONLINE - On the summary page, there is a link near the bottom to Register another Player. Click on this link to ensure that all your family's players are registered at the same time.
- OFFLINE - Please print as many registration forms as you need and mail them at the same time.
Failure to register all players in your family at once will result in you being charged the full rate for each player which could result in you paying more than the $500 family limit!
Optional Payment Plan
New this year! If you register online for the Classic and/or RecPlus levels, you will have the option of enrolling in a payment plan. An initial fee of $50 (per player) will be billed at time of registration. The balance will be divided equally to be billed on April 1 and May 1.
Families at the family maximum are not eligible for the the deferred payment plan.
Late Registration Fee
LATE REGISTRATION FEE: A late registration fee will be added to all players registering after the dates posted above. This late fee also applies to those who would normally reach a family maximum of $500 resulting in a payment of more than $500 for their family. There will be no exceptions to this late fee.
To avoid the late fee please register on time. This helps in planning for teams as late registrants cause many team formation difficulties.
Emergency Contact Information and Consent Form
Please print off and mail in the following Emergency Consent Form. This form MUST be turned in before your child will be allowed to practice or play in any games.

We prefer you to register online; however, if you choose to not register online you will need to print off and mail in this form along with your completed Emergency Consent form located above along with your registration fee to:
