Spring Soccer 2008

Required information for registrants:

  •  Copy of birth certificate (if not already on file with NUAYSA)

  •  Health Insurance Policy Number

  •  Doctor/Dentist phone numbers

  •  Emergency contact Information (including primary & secondary
     phone #s and email contact information)

  •  Uniform Size Information  

Registration forms will be available on Registration Day or by clicking the links below for printable versions of both the registration form and the Emergency consent form.**  Registration fees are non-refundable.

Here's our Program.....

REC   Recreational League:  Up to U8 teams only.  No travel. Teams may be co-ed.  Practices and intramural games generally held Tues/Thurs evenings.  Registration cost is $65    Fees include T-shirt and socks and a fundraising component.  

REC-PLUS   Recreational-Plus League:  U10 only.  Travel for games. Practice and game days may be Mon/Wed or Tue/Thurs, with some games on weekends. Teams are gender-specific, unless there are not enough kids in an age group. Registration cost is $175   Again, fees include a uniform and a fundraising component. 

CLASSIC   League:  U12-U19.  Travel for games. Teams are gender-specific, unless there are not enough kids in an age group. Registration cost is $225  and includes both a home and away uniform, tournament fees and a fundraising component. 

  • Family maximum of  $450 for the Spring Season   
  • Fees are Non-Refundable
  • A $40 Late Fee is assessed to registrations that come in past January 31

Required equipment for all leagues:

Uniform
 

Appropriate Shoes

Water bottle

Shin Guards

Socks that cover shin guards

Soccer ball of correct size*

**Mouth Guards are  Recommended

**No jewelry of any kind will be allowed. 

**Eyeglasses must be secured with
    safety straps.

We Need You!!!

Parents and Volunteers:  We are proud to be a parent-run organization. We rely on parent/adult participation to succeed in our goal of offering an excellent soccer program.  A minimum of 10 volunteer hours   per player   are mandatory during the Spring Season.  Families may “buy-out” their volunteer hours for $100.  A family maximum of 20 hours and $200 buyout fee applies.  Those who do not fulfill their obligation will be billed.
